Family Membership (3 or more individuals residing in the same location)- Enrollment      $120

Enrollment fees are a one time cost and include the transfer of your medical records from any medical practitioners you may have been previously associated with. Our doctor will review your medical records and ensure that direct primary care is the right fit for your medical needs. If we aren't able to provide for your medical needs, we will refund all of your enrollment fees. 

Enrollment also includes access to our patient portal. Our patient portal gives you 100% access to your medical records 24/7. If you travel and have an emergency, you will have the ability to share these records with hospital doctors immediately. We believe that patients who are informed are better able to make important health decisions.

All monthly membership fees include the following:

We also offer extended visits so your appointment time is the time you will be seen in our office with little to no wait time. This means no more missing work to sit in a waiting room.
• 24/7 access to an actual doctor for urgent matters. If your toddler has a high fever at 2am and you aren't sure what to do, it's great to know that you can call and speak to your child's doctor instead of an answering service. 
• Unlimited visits For people managing a chronic health issue like diabetes or asthma or for individuals with a high deductible, the frequent visits to the doctors office can get very expensive. With Elevated Healthcare, you know that, for your monthly membership fee, you will be able to see the doctor as often as you need to.
• Convenient  Online Scheduling Through our online portal you can manage your appointments, medication refills, and even conduct telemedicine appointments securely and privately with your doctor from the comfort of your own home or office. We offer you a easy to operate platform that works on all device platform.
